2011-2012 Season
PRE-REGISTRATION FOR THE 2011-2012 SEASON..
ATTENTION :
For your registration to be valid, you need to send it with all the necessary documents to validate it. After August 31st 2011, refer to the Payment Schedule document since there will be supplements !
Necessary documants
The completed Payment Schedule and your payment
(Do not forget to make your check to the order of AHMVD for Vaudreuil-Dorion residents, and AHM LES CÈDRES for Les Cèdres residents)
A copy of your child’s Assurance maladie card
Proof of residence to receive each city’s subsidy, and for Hockey Québec (copy of the first page of the school report card)
A RECEIPT WILL BE PREPARED WITH THE NAME LISTED ON THE PAYMENT FORM. PLEASE INDICATE IT CLEARLY. PLEASE NOTE THAT THIS RECEIPT CANNOT BE SPLIT AND HAS TO BE DONE TO THE NAME OF EITHER PARENT, OR TO BOTH.
The members of Les Cèdres register with the AHMVD registrar, but the cheque has to be done to the order of AHM LES CÈDRES.
Junior players also have to registrer with their respective associations. There are no subsidies at that level.
Girls need to register with their respective associations.
Below, you will find the necessary documents to register for the 2011-2012 season. Please advise us of any change of address by sending the registrar an email.
Eligibility for on-ice activities
Players need to be officially registered to attend tryouts for AHMVD and Trois-Lacs, training camps and any other on-ice activities. Unregistered players won’t be admitted, without any exception. Players need to register during the official registration period.
Double letter players
Each double letter player that moves to a new association/organization needs to get a release from his current association before registering with the new one. Consult the website (www.ahmvd.ca) mid-August to get details on double letter Tryouts and AHMVD regular training camps!!

Training camp jerseys
Attention ! Please note that an amount of $15.00 will be asked from each player before handing them their training camp jersey. Each player will keep the jersey at the end of camp. We ask that the amount be paid in cash, and preferably the exact amount.
Income tax receipts
Receipts will be distributed at the end of February.
